This form is for external providers to refer whānau to Hapū Wānanga ki Taranaki services, including but not limited to whānau engagement services and kaupapa such as Hapū Wānanga, Restore Your Core, Playgroups, The Aunties, and car seat hire, installation and advice. Once a referral has been received, Hapū Wānanga ki Taranaki will make contact with the referred whānau. Further registration and consent will be obtained directly from the whānau before services are provided.
